I agree to information about me collected by the New Zealand Defence Force being used for the following purposes:

I also agree to the following members of the New Zealand Defence Force having access to this information:

• Recruiting Staff
• Defence Psychologists
• Medical Staff
• Training Staff
• Administration Staff
• Commanders.

I understand this information will be held securely and that the New Zealand Defence Force will not disclose it to any person except for the purposes described above, or as authorised by myself, or by law.

I also understand that this information will be held by the New Zealand Defence Force until such time as it is no longer required.

I authorise the collection of information about myself from referees that I have nominated in respect of my recruiting application.

I agree to information being collected on my character and academic ability from employers, schools or other educational institutions, and from the Police.

I understand that I may request access to, and correction and updating of any information that is collected about me by the New Zealand Defence Force. I also understand that access may be limited in the case of evaluative information.
